Applicant operation flow chart
(1) Application qualifications:
1. Students of this school.
2. Faculty and staff of the school (including retirees and their families).
3. Alumni of this school.
4. Social people.
Note: Dependents include spouse and immediate relatives (excluding relatives of spouse)
(2) Application procedures:
1. Fill out the application form and pay the fee.
2. Application method: fill in the form through the online system. (Enter: School Affairs Information System)
3. Certificate collection and payment location: Alumni Gymnasium.
Note: In order to promote the convenience of payment for venue users and implement e-management of venues, starting from September 1, 2020, the payment method for personal use certificates and borrowing of sports venues will be changed to "Easy Card" deduction, and no cash will be accepted.
(3) Instructions for use:
1. The license is valid for a limited time and will become invalid upon expiration.
2. After payment, refund, retention, transfer or extension cannot be requested for any reason.
3. The license cannot be altered without authorization or transferred to others for use. In case of violation, the license will be confiscated and invalidated, and applications will be suspended for one year.
4. If the license is lost or damaged, you should report the loss immediately and apply for a new one (cost of NT.100 ).
5. For parking on campus for those who apply for a pass:
(1) On-campus: students, faculty and staff follow on-campus parking regulations.
(2) Alumni: Follow the alumni parking discounts.
(3) Off-campus visitors: Follow on-campus parking regulations.
